This is the information on another vaccination site :Agri~Plex facility in Allentown.

The site is open Monday-Friday located at the Allentown Fairgrounds off of Liberty Street. APPOINTMENTS ARE REQUIRED. More dates may be added starting on Thursday - Please check back often on the Health Dept site. (see link below) Their goal is to vaccinate 500 people there per day. Proof of age is required.

Location: Agri-Plex at Allentown Fairgrounds, 302 N. 17th Street, Allentown, PA 18104 Date: Multiple Dates (NOTE: If no dates are listed below than all the vaccine appointments have been filled.) The Allentown Health Bureau is receiving a limited supply of Moderna COVID-19 vaccine from the Pennsylvania Department of Health. Appointments are required. The Allentown Health Bureau can NOT vaccinate under 18 years of age because the Moderna vaccine is not approved for those 16 and 17 years of age. IMPORTANT - DO NOT schedule an appointment if: You are NOT in the expanded Phase 1A category. You will be turned away at the vaccination site if you are ineligible. You are due for the 2nd COVID vaccine but DID NOT get the 1st vaccine from the Allentown Health Bureau. NOTE: You must get the 1st and 2nd COVID vaccine from the same provider. Upon scheduling your appointment, you will be taken to a confirmation page. Print out or write down your appointment date, time and location before closing the confirmation page. NOTE: If no dates are listed below than all the vaccine appointments have been filled. If you select a date but there are no appointment times listed, than all the appointment times have been filled. As healthcare workers, employees can follow the following steps to get into the queue to receive their first doses.

  1. Sign in to MyLVHN or sign up for MyLVHN. Call 888-402-LVHN (5846) for help setting up an account. Links to sign up or sign in are available on LVHN.org/vaccines.

  2. When you are logged in to MyLVHN, click “Menu,” “Essential Worker Questionnaire” and complete the form. Important note: You may not see your organization in the Healthcare industry and that is OK. I encourage you and your employees to select “Not Listed” in your Industry classification to move through the questionnaire. Just because your organization is not listed doesn’t mean that you and your employees cannot get a vaccine through LVHN.

  3. Once you complete the form, you will be notified through MyLVHN, email or text (depending on your notification preferences) within TWO HOURS or less when you can self-schedule the date and location of your vaccination in MyLVHN.

  4. You also can schedule an appointment by calling 484-884-1767.
