We have emailed everyone the new EVV policy revised; take a moment to read and sign. If you have a problem with your email, make sure to reach out to Yenica Buskirk.
There will be a video on it for further training if necessary.
With it, we send a Self-Monitoring Acknowledgement form. Make sure you read it. Once sign and received at the office, you will no longer need to fill out the monitoring forms and send them in.
